Driver Medical Form

Forms are completed Thursday to Sunday between 10:00 AM and 6:00 PM in South Surrey.

1. Text your name and the time you would like to come to  604-449-4566.
2. You will receive a confirmation text.
3. Arrive for you appointment.
4. Pay $125 via cash or e-transfer after seeing the doctor.

All forms are faxed immediately to RoadSafetyBC. You keep the original form.

Who?

A person must complete a Driver’s Medical Fitness Examination Report when they apply for a:  Class 1, 2, 3 or 4 licence Class 5 licence with endorsements 18, 19, 20.

This includes Taxi/Uber/Lyft drivers, short haul and long haul truck drivers.
